Surgical correction; Surgical correction of the hypertropia is desired to achieve binocularity, manage diplopia and/or correct the cosmetic defect. Steps to achieve the same depend on mechanism of the hypertropia and identification of the offending muscles causing the misalignment.
Adult and child observers perceived a right heterotropia as more disturbing than a left heterotropia, and child observers perceived an esotropia as "worse" than an exotropia. Successful surgical correction of strabismus, for adult as well as children, has been shown to have a significantly positive effect on psychological well-being.
There is tentative evidence that children with infantile esotropia achieve better binocular vision post-operatively if the surgical treatment is performed early (see: Infantile esotropia). A recent study reported the reoperation rate in a sample of over 6000 patients being 8.5%.
When this muscle's function is diminished due to a fourth cranial nerve ( CN IV) palsy, the affected eye will extort, deviate upward ( hypertropia ), and, to a smaller extent, drift inward.
The prism cover test (PCT) is an objective measurement and the gold standard in measuring strabismus, i.e. ocular misalignment, or a deviation of the eye. It is used by ophthalmologists and orthoptists in order to measure the vertical and horizontal deviation and includes both manifest and latent components. [1]
For an abnormal result, based on where the light lands on the cornea, the examiner can detect if there is an exotropia (abnormal eye is turned out), esotropia (abnormal eye is turned in), hypertropia (abnormal eye higher than the normal one) or hypotropia (abnormal eye is lower than the normal one).
Prism correction is measured in prism dioptres. A prescription that specifies prism correction will also specify the "base". The base is the thickest part of the lens and is opposite from the apex. Light will be bent towards the base and the image will be shifted towards the apex.
Hypertropia/Hypotropia. In a patient with an unsuppressed vertical deviation, one line will appear higher than the other. If the image of the right eye is higher than that of the left, this means the right eye is lower than the left. This could be either a right hypotropia or a left hypertropia. Far-sightedness primarily affects young children, with rates of 8% at 6 years old and 1% at 15 years old. It then becomes more common again after the age of 40, known as presbyopia, affecting about half of people. The best treatment option to correct hypermetropia due to aphakia is IOL implantation.
Some consider botulinum injections to be a treatment option for children with small- to moderate-angle infantile esotropia. [7] [8] Studies have provided indications that performing injections into both medial rectus muscles may be more effective than an injection into one medial rectus muscle alone.
