The caption to the photo states that the person damaged the knuckles "via the mouth" - which presumably means biting or chewing, not picking with the fingers (hence the label dermatotillomania seems inappropriate). I have exactly the same calluses on my knuckles from nervous chewing of my fingers - and I understand that to be dermatophagia.

Admittedly, I do sometimes pick at the callouses on my hands, but they are (as far as I can tell) 95% the result of biting or chewing. Also, I have seen dermatophagia and related disorders described as obsessive-compulsive spectrum disorders, even though they are not part of the classic obsessive-compulsive disorder (in the narrow sense).
