Dermatophagia. Dermatophagia (from Ancient Greek δέρμα — lit. skin and φαγεία lit. eating) or dermatodaxia (from δήξις, lit. biting) [3] is a compulsion disorder of gnawing or biting one's own skin, most commonly at the fingers. This action can either be conscious or unconscious [4] and it is considered to be a type of pica.
Nail biting, also known as onychophagy or onychophagia, is an oral compulsive habit of biting one's fingernails. It is sometimes described as a parafunctional activity, the common use of the mouth for an activity other than speaking, eating, or drinking. Lesch–Nyhan syndrome. Lesch–Nyhan syndrome ( LNS) is a rare inherited disorder caused by a deficiency of the enzyme hypoxanthine-guanine phosphoribosyltransferase (HGPRT). This deficiency occurs due to mutations in the HPRT1 gene located on the X chromosome. LNS affects about 1 in 380,000 live births. [3]

The caption to the photo states that the person damaged the knuckles "via the mouth" - which presumably means biting or chewing, not picking with the fingers (hence the label dermatotillomania seems inappropriate). I have exactly the same calluses on my knuckles from nervous chewing of my fingers - and I understand that to be dermatophagia.
Muscles of mastication. Mandibular division of the trigeminal nerve. The four classical muscles of mastication elevate the mandible (closing the jaw) and move it forward/backward and laterally, facilitating biting and chewing. Other muscles are responsible for opening the jaw, namely the geniohyoid, mylohyoid, and digastric muscles (the lateral ...
